Practice Nurse
Royal Manor Health Care – Jurassic Coast, Dorset | 24hrs per week to be worked Wednesday to Friday | Band 5/6 (depending on experience)
We are a welcoming and supportive GP surgery at the heart of our community, and we’re looking for a caring and motivated Practice Nurse to join our team.
This is a great opportunity for a nurse who enjoys building relationships with patients and wants to make a real difference in long-term condition management (such as diabetes, asthma, COPD, and cardiovascular health), while also being involved in everyday practice nurse work.
What the role involves
- Supporting patients to manage their long-term health conditions with confidence.
- Carrying out a variety of clinics, including immunisations, cervical screening, wound care, and NHS Health Checks.
- Helping patients stay well with health promotion and lifestyle advice.
- Working closely with GPs, nurses, and the wider team to deliver joined-up care.

Job Purpose
The Practice Nurse will work as part of the multidisciplinary team within the GP surgery, delivering high-quality, evidence-based nursing care to patients. The post holder will provide general practice nursing services with a particular focus on the management of long-term conditions (LTCs) such as diabetes, asthma, COPD, and cardiovascular disease. They will support health promotion, disease prevention, and proactive patient care within the framework of primary care.
Key Responsibilities
Clinical Duties:
- Provide nursing care and treatment to patients in accordance with evidence-based practice, clinical guidelines, and surgery protocols.
- Support and manage patients with long-term conditions including (but not limited to): diabetes, hypertension, asthma, COPD, and cardiovascular disease.
- Undertake routine and follow-up reviews for LTC patients, ensuring care aligns with QOF and local pathways.
- Carry out , wound care, cervical cytology, immunisations (childhood and adult), travel vaccinations, work at leg club and other general practice nursing duties.
- Promote patient self-management and shared decision-making in managing LTCs.
- Recognise and act on early signs of deterioration in patients with chronic conditions.
- Work collaboratively with GPs, Advanced Practitioners, and wider MDT to support continuity and quality of care.
- To work at our Island Leg club as needed.
Health Promotion & Prevention:
- Provide lifestyle advice on diet, exercise, smoking cessation, alcohol reduction, and weight management.
- Support proactive care planning for at-risk and vulnerable patients.
- Participate in health campaigns and screening initiatives within the practice population.
- To facilitate evening education as and when required.
Professional & Governance Responsibilities:
- Maintain accurate, timely, and legible patient records using the clinical system.
- Adhere to infection prevention and control policies and promote safe practice.
- Participate in clinical audits, significant event analysis, and quality improvement activities.
- Maintain professional registration with the NMC and engage in ongoing CPD.
- Contribute to the training and mentorship of junior staff, students, and new team members.
Person Specification
Essential:
- Registered Nurse (NMC).
- Experience working in a primary care or community setting.
- Knowledge and/or experience in managing long-term conditions (e.g. diabetes, asthma, COPD, CVD).
- Competence in immunisations, wound care, cervical cytology, and general treatment room skills.
- Strong communication, organisational, and clinical decision-making skills.
- Commitment to patient-centred care and health promotion.
Desirable:
- Previous experience as a Practice Nurse in general practice.
- Post-registration training in LTC management (e.g. asthma/COPD diploma, diabetes management).
- Experience of using GP clinical systems (e.g. EMIS, SystmOne).
- Mentorship or teaching qualification.
